In order to process your request and reserve a guide for your program we will need each member of your group to fill out our Registration Form. Please be sure to fill it out completely and accurately. For your convenience, you may type directly into the fields below and send it back to us by email, or feel free to print this form, fill it out by hand, and then mail it back to our office.
We must receive this form and at least a 50% deposit in advance to process your booking and to confirm your guide and a spot in one of our programs. If registering within 60 days of the trip start date, the full amount is due.

Mountaineering, Rock Climbing and Ski Mountaineering are dangerous, demanding activities and are very serious undertakings. Many of our programs take place in remote settings where rescue is difficult and definitive care is far away. Additionally, most of our programs require a relatively high level of physical activity for a considerable length of time. Being physically fit is very important, and can make dealing with hazards less dangerous. Thank you for taking the time to accurately answer all of the questions on this form.
